Privacy Notice
Who this covers
This notice applies to the driver pre-qualification form on this website, operated by On Demand Transportation Group LLC (“ODT”, “we”). It does not cover any separate application, screening, or onboarding process you may be invited into later, each of which carries its own notices and authorizations.
What we collect
When you complete the pre-qualification form we collect:
- Contact and identity: your name, email address, mobile phone number, date of birth, and home address.
- Licence: your commercial driver's licence number, class, issuing state, expiration date, and endorsements.
- Experience and safety history: years of commercial driving, equipment operated, and your self-reported counts of accidents and moving violations in the last three years.
- Credentials: whether you hold a current DOT medical examiner's certificate, whether you are registered with the FMCSA Drug & Alcohol Clearinghouse, and whether you hold a TWIC card.
- Employment history: the employers you list, with dates, position, reason for leaving, and whether each role was DOT safety-sensitive.
- Your authorizations: which consents you agreed to, the exact wording you were shown, your typed signature, and the date and time.
- Technical: your browser's user-agent string, and your IP address in a one-way hashed form used to limit abuse of the form. We do not store your IP address itself.
We do not ask for, and the form does not accept, your Social Security number, bank or payment details, tax forms, medical records, or drug-test results.
Why we collect it
To decide whether you are a potential fit for an advertised driving role, to contact you about your application, and to prepare a complete packet for the carrier that makes the qualification decision. We do not use this information for advertising, and we do not sell it.
Who we share it with
- QMJ Logistics, LLC, the carrier of record, for carrier qualification, compliance checks, and safety-eligibility decisions. ODT does not make those determinations.
- Screening and consumer-reporting providers, where you have given a separate authorization for a motor vehicle record, a Pre-Employment Screening Program report, or a consumer report.
- Service providers that run this website: our hosting platform (Vercel), our database provider (Neon), and our email provider (Resend). They process this information on our behalf and are not permitted to use it for their own purposes.
We may also disclose information where the law requires it.
How it is protected
- The site is served over HTTPS, and the database is encrypted at rest.
- Your CDL number is separately encrypted before it is stored. Only the last four digits are held in readable form.
- Your IP address is salted and hashed, never stored in the clear.
- Our application logs and internal notification emails deliberately exclude your licence number, date of birth, street address, and signature.
How long we keep it
We keep pre-qualification records for as long as we need them to evaluate your application and to meet our legal and regulatory obligations, which differ for hired drivers, driver qualification files, consumer-report records, and applicants who are not selected. Records are not automatically deleted, and you can ask us to delete yours as described in Your choices below.
Your choices
Completing the form is voluntary. You can ask us what we hold about you, ask us to correct it, or ask us to delete it, and we will honour that request unless a legal or regulatory obligation requires us to keep it. Depending on where you live, you may have additional rights under your state's privacy law.

Changes
If this notice changes we will publish a new version here with a new version identifier. The version you agreed to is recorded with your application, so a later revision does not change what you consented to.
